Description: This patient had severe breathing issues as well as wanting to improve the crooked appearance of his nose. By reconstructing his septum, I was able to straighten the nose, improve his breathing, and also refine the overall appearance of his nose.

Description: An upper lip lift was performed to improve the fullness and shape to the lips. She previously had filler and didn’t like the temporary nature and overall look that it gave her. She is very pleased that she has a permanent option to a small upper lip.

Description: Patient did not like the lack of upper lip that she had and wanted something more permanent to shorten the upper lip. An upper lip lift was performed to improve the shape and show of the red lip.

Procedure Detail: Patient came for consultation with Dr. Somenek because she had a large dorsal hump and a drooping tip. In addition, she had considerable nasal obstruction that was affecting her ability to breathe when she exercised. Dr. Somenek performed a septoplasty to straighten the nasal septum, thus improving her ability to breathe through the nose. Cartilage grafts(alar rim grafts) were placed along the nostril margin to prevent them from collapsing during inspiration. This opened up her airway and alleviated any nasal obstruction that she had in the past. Her cosmetic concerns were also addressed which included removing the dorsal hump with an osteotome and refining her nasal tip.

Procedure Detail: This male presented with an interest in improving the shape of the nose as well as improve his breathing. Specifically, he desired removal of a dorsal hump (bump on the top of the nose) and correction of a droopy tip. Dr. Somenek performed a septoplasty to straighten the nasal septum, thus improving his ability to breathe through the nose. The dorsal hump was shaved down to smooth the profile. Nasal tip rotation was achieved with tip narrowing sutures and a columellar strut graft was placed using cartilage from the nasal septum to provide long lasting structural support. This will prevent the tip from drooping in the future.

Procedure Detail: Female patient presented to Dr. Somenek in consultation for a rhinoplasty. She was interested in refinement of the nasal tip and narrowing the width of the nose.
In an effort to achieve tip refinement, Dr. Somenek performed cephalic margin trims to address the broad width of the cartilages. Then, various suture techniques(interdomal, transdomal and tip position sutures) were used to further refine the nasal tip shape. Osteotomies were performed to narrow the nasal bones and create more definition to the appearance of the nose.

Procedure Detail: Male patient came to Dr. Somenek with the primary concern of a droopy tip. Additionally, he did not like the hump on his nose. Dr. Somenek performed rotation to the nasal tip. This also involved placing a cartilage graft called a columellar strut to provide increased support to the nasal tip so it would not droop in the future. The profile was shaved down. An additional cartilage graft was placed in the radix(most superior part of the profile) to create a stronger profile. The use of cartilage grafts create a long lasting, stable result for years to come.
Procedure Detail: Female patient presented with an interest in improving not only the function of her nose but also the shape. Specifically, she desired removal of a dorsal hump (bump on the top of the nose). Dr. Somenek performed a septoplasty to straighten the nasal septum, thus improving her ability to breathe through the nose. Dorsal hump reduction with a rasp was then used for refinement of the bony nasal dorsum. Cartilaginous dorsal hump was removed conservatively. The upper lateral cartilages were preserved to further maintain the integrity of the middle nasal vault and internal nasal valve. Nasal tip support was provided with a columellar strut.

Procedure Detail: Female patient came to see Dr. Somenek because she has been always bothered by her nasal tip asymmetry. There was no history of trauma but she felt that it was worsening with age. Dr. Somenek performed suturing techniques to narrow and refine the nasal tip. He also performed osteotomies to narrow the nasal bones. These techniques helped to achieve an improved shape and symmetry to the nose.

Procedure Detail: Female presented with concerns about her dorsal hump and the appearance of her profile. Dr. Somenek performed a rhinoplasty to remove the bump and bring the nose closer to the face(deprojection). This is achieved with advanced suturing techniques that involve repositioning the cartilaginous structures that make up the nasal tip. Her result demonstrates a more refined appearance that harmonizes with the rest of her facial features.
